A. New passports (First time Applicants)

Requirements:

  1. A dully filled and signed Form ‘A’.
  2. \r\n
  3. A dully filled and signed Form ‘B’/89.
  4. \r\n
  5. A Completed Consular Registration Form. (If not previously submitted)
  6. \r\n
  7. Passport size photographs as decided in the section of Form ‘A’.
  8. \r\n
  9. Application fee: RMB 400-
  10. \r\n
\r\n

B. Renewal of Passports

\r\n

Holders of Passports that have expired or are used up are now required to apply for new Passports. A form should therefore be completed as in ‘A’ (1) above.

\r\n

Forms can be picked from the Embassy in Beijing.

\r\n

C. Passports for Children

\r\n

Requirements:

\r\n

1) A dully filled and signed Form ‘A’.
\r\n 2) A dully filled and signed Form ‘B’/89.
\r\n 3) Completed Consular Registration Form. (if not previously submitted)
\r\n 4) 2 recent passport photographs.
\r\n 5) Copy of Birth Certificate, in case the child was born outside Uganda.
\r\n 6) An application fee: RMB 400-

D. Change of holders particulars on passports

\r\n

Requirements:

\r\n

1) An affidavit or Marriage Certificate
\r\n 2) Dully filled form ‘A’.
\r\n 3) 2 Passport size photos.

\r\n

Required alterations will be done by the Passport Office. A new passport will have to be issued.

\r\n\r\n

E. Travel Document (Emergency)

\r\n

Requirements

\r\n

1) A dully filled Form ’A’.
\r\n 2) A dully filled Consular Registration Form (if not already registered with the Embassy.
\r\n 3) Three (3) recent passport size photographs
\r\n 4) An application fee of RMB 200
\r\n 5) In case of loss of Passport:
\r\n I. A Police report.
\r\n II. A hand written record of the circumstances surrounding the loss of passport.
\r\n III. A dully filled Form ‘A’.
\r\n IV. Copy of lost Passport if available.
\r\n 6) Applicants may be required to provide additional supporting documentation.

\r\n

Note:

\r\n

1) An Emergency Travel Document is a one-way travel document to Uganda and it can only be issued after exhaustive inquiries have been made. Passports reported lost will be declared null and void.
\r\n 2) Application for an Emergency Travel Document for Minors (under 16) must be accompanied by:
\r\n a) A written consent from both parents.
\r\n b) Copies of Passports of both parents.
\r\n c) Original Birth Certificate.
\r\n d) 2 passport photographs of the minor.
\r\n e) Dully filled forms ‘A’ and ‘B’. Only form ‘A’ of a minor who has previously held a Passport.

F. Replacement of Passports (Lost Passports)

\r\n

All payments should be made at the time of application. The fee, once paid and receipted is not refundable.

\r\n

Specimen Signature: Applicants are advised to append their specimen signature in the centre of the box. Use of a black pen enhances the clarity of the specimen signature.

\r\n

Endorsement: Every applicant including children will have to apply separately for a new passport. A thumb print is required for children who cannot write.

\r\n

ALL APPLICANTS WHO ARE UNABLE TO COLLECT THEIR PASSPORTS PERSONALLY FROM THE EMBASSY MUST ARRANGE IN ADVANCE FOR COURIER SERVICES TO HAVE THEIR PASSPORTS PICKED FROM THE EMBASSY AND DELIVERED TO THEM.

Note:

\r\n

Applicants are advised to confirm with the Embassy whether their passports are ready for pick-up, before sending /notifying the relevant courier to collect them.
